We believe in a transparent Fee Schedule so you can plan ahead and determine what your health is worth.

  • $375 // 1.5 hours telehealth appointment

    We get to know the real you. Includes extensive review of health history, current symptoms, medication review, telehealth appropriate physical exam, review of any pre-existing labwork, plan of treatment including discussing future comprehensive labwork, imaging, goal setting and supplement/diet plan.

    Includes a 15% First order discount on Fullscripts- applied at the follow up visit

    *Wisconsin Clients only receive Nurse Practitioner Services

    **Nurse Coach in COMPACT states fee $275

    ***Comprehensive functional lab tests and/or imaging incur separate fee

  • Follow Up Visit

    $275 //1 hour telehealth appointment

    Essentially part 2 of a New Patient visit (or for established patients-follow up after annual functional medicine lab review.)

    This appointment will be scheduled once all pertinent lab work and imaging is back for review. A personalized treatment plan which includes lifestyle adjustments, supplement, medication (where necessary), and dietary recommendations.

    (Or any annual treatment and/or goal adjustments)

  • $175 // 30 min telehealth appointment

    Brief concerns addressed (ones deemed too complex for messaging)//Med check dose adjustment//Medication refills//Simple lab follow up.

  • $285 // 1 hour telehealth appointment for established patients.

    Annual review and adjustments to goals/plan set out in previous visits. Includes review of health history, current symptoms, medication review, telehealth appropriate physical exam, scheduling of new comprehensive lab

  • Think of this as your integrative health concierge medicine.

    Very Important Patient Membership requires a 12 month commitment. A valid credit card will need to be on file.

    This includes:

    • 1 annual or new patient telehealth visit ($375 for 1 hour) + 1 annual follow up ($285 for 1 hour)

    • 1 Free -4 major food allergens test eg Milk, Soy, Dairy, Gluten.

    • 1 year of patient discounts on all Fullscript supplements of 15%.

    • Priority scheduling for appointments.

    • 6×30 minute check ins (for medication adjustments/simple lab follow up etc).

    • Any further appointments/visits longer than 30 min at a discounted rate- 15% discount.

    *New Patients may opt for this option, however, a one time administrative fee of $100 will be added.

    **Pre Pay for 12 months of membership receive 10% off: $1650/year

  • -Students receive 15% off for New Patient Visit and follow up only. Must send valid and current school ID. Labs/imaging not included with discounted rate.

    -Genuine need-based aid// Please contact us via email or message with a brief essay on your situation/finances, your health struggles and goals for an appointment at ASW. We will get back to you whether you have been approved for a discounted rate. Unfortunately any lab work/imaging/medication/supplements cannot be discounted.

  • We are a cash-based, fee-for-service small business. We do not accept insurance. You may be eligible to use your HSA/FSA dollars.

    Currently accepting credit/debit cards and PayPal.

  • We are currently offering telehealth appointments only.

    Full Nurse Practitioner medical services are available for patients located in Wisconsin at this time (Illinois coming soon-Stay tuned!).

    If you live outside of Wisconsin, there are still ways we can work together. Through Holistic Nurse Coaching services, we are able to serve clients in the following Nursing Compact (eNLC) states: Alabama, Arizona, Arkansas, Colorado, Delaware, Florida, Georgia, Idaho, Indiana, Iowa, Kansas, Kentucky, Louisiana, Maine, Maryland, Mississippi, Missouri, Montana, Nebraska, Nevada, New Hampshire, New Mexico, North Carolina, North Dakota, Ohio, Oklahoma, South Carolina, South Dakota, Tennessee, Texas, Utah, Vermont, Virginia, West Virginia, and Wyoming.

    As part of our Nurse Coaching services, we can also assist you in setting up comprehensive and integrated lab testing to support your personalized wellness journey.

  • Please note that laboratory testing and imaging services are billed separately from your consultation fee.

    We are committed to helping you find the most cost-effective options available. For most patients, comprehensive initial lab work typically ranges from $500 to $850 or more when paying out-of-pocket.

    In some cases, there may be limited opportunities to utilize insurance coverage for certain labs or imaging studies. Please inquire with our office prior to your appointment if you would like more information.

    *Final lab costs vary depending on the complexity of your case and personalized recommendations.

    • Once you book, you’ll receive a e-mail to register to the PHR or patient health record (patient portal). Please register so that you are able to access the secure electronic intake forms to complete before your appointment. These cover your medical history, symptoms, lifestyle, and health goals — helping us create a truly personalized plan.

    • You will be asked to review and sign informed consent documents for treatment, telehealth services, and privacy practices (HIPAA).

    • Copy of Driver’s License: For your safety and to comply with healthcare regulations, we require a copy or a picture of your driver’s license or government-issued ID prior to your appointment.

    • Upload Prior Records: If available, please upload any relevant laboratory results or imaging studies from the past 1–2 years into your patient portal under the “Documents” section. This information helps us better understand your health history and prevents unnecessary repeat testing.

    • Medication and Supplement List: Please have an up-to-date list of all medications, supplements, and vitamins you are currently taking.

    • Health Goals: Take a moment to reflect on your top health concerns and goals so we can prioritize what matters most to you.

    • This whole process if done thoughtfully may take up to an hour or more of your time. Please plan ahead to have this done in a timely at least 48 hours before your first apppointment.
